Chronic Lung Disease of Prematurity in twelve clinical teaching cases with worked explanations. Each case gives you a clinical vignette, four options, the correct answer, and an explanation of why each of the other three is wrong.

The 12 cases, in order

  1. The day the diagnosis is made

  3. Why the lung stopped growing
  4. Reading the film
  5. The interventions that changed the disease
  6. Breathing instead of growing
  7. Oxygen at home
  8. The heart that gives it away
  9. Working harder than yesterday
  10. The winter that decides the year
  11. Going home
  12. The lungs at school

Diagnoses and management options tested

Across the twelve cases you are asked to choose between options such as: Whether he received surfactant, Widespread hyaline membranes lining the alveolar ducts, Thickened, muscularised small airways with mucus plugging, Acute infection superimposed on a normal lung, Retained fetal lung fluid, Established chronic lung disease of prematurity, Pulmonary interstitial emphysema, Early caffeine, Routine diuretic therapy from the first week, Inadequate prescribed feed volume, Cows milk protein allergy, Undiagnosed coeliac disease, A sweat test, A barium swallow, An echocardiogram, A high-resolution chest computed tomogram, The initial assessment overestimated the severity, Immunoprophylaxis against respiratory syncytial virus, Inhaled corticosteroids started at discharge, Keeping her indoors until the spring, Return only if the oxygen equipment fails.

Clinical pearls from this deck

  • The diagnosis is an assessment on a date, not a summary of a bad fortnight. Oxygen for 28 days, graded by what is needed at 36 weeks corrected.
  • Positive pressure trumps concentration. A baby on CPAP in 21 per cent oxygen has severe disease.
