The 12 cases, in order

  1. Well between the colds
  2. The noise the parents mean
  3. How old is this child
  4. The first twenty minutes
  5. Spacer or nebuliser
  6. How sick is this one
  7. Who gets the steroid
  8. The wheeze that never fitted
  9. It started in one second
  10. Trying a preventer properly
  11. Will this become asthma
  12. Before they go home

Diagnoses and management options tested

Across the twelve cases you are asked to choose between options such as: Episodic viral wheeze, Multiple-trigger wheeze, Recurrent bronchiolitis, Wheeze from small airway narrowing, Coarse secretions in the large airways, Stridor, arising from the upper airway, Asthma presenting early, Inhaled foreign body, Intravenous salbutamol, A chest radiograph before any treatment, Agree, and arrange a home nebuliser, The first assessment overestimated the severity, Investigate for an underlying diagnosis, Double the dose to consolidate the improvement, A three-month follow-up appointment on its own, A chest radiograph before he leaves.

Clinical pearls from this deck

  • Ask what the child is like between the colds. That answer, not the severity of the attack, names the phenotype.
  • Ask the parent to imitate the noise, or to play a recording. The word wheeze covers several sounds with different causes.
