What this deck covers

What this deck teaches
- A new infiltrate plus one respiratory finding. That is the definition. Without a film you have not excluded it.
- The first film is often clear. The infiltrate appears on day two or three. Repeat it when the child deteriorates.
- It is rarely one cause. Infection, infarction, fat embolism and hypoventilation, usually together.
- The route in runs through shallow breathing. Rib pain and opioids cause splinting, then atelectasis, then sickling.
- Incentive spirometry on admission prevents it. Cheap, safe, and the thing most often prescribed and never done.
- Cephalosporin PLUS a macrolide. Leaving out the macrolide is the classic prescribing error.
Exam pearls from this deck
- A new infiltrate plus one respiratory finding. If you have not filmed the chest, you have not excluded it.
- A clear film on admission does not exclude acute chest syndrome. The infiltrate usually appears on day two or three.
