Why the other options are wrong
Case Complete
What this deck covers
Inflammatory Bowel Disease in Children in twelve illustrated clinical cases with worked explanations – case presentations built around clinical images, radiology, teaching illustrations and real laboratory data, with the reference range beside every value a case quotes. This visual, data-interpretation style of practice suits candidates preparing for MRCPCH AKP in particular, alongside Saudi and Gulf licensing examinations such as the Arab Board, DHA and OMSB. Each case gives you a clinical vignette, four options, the correct answer, and an explanation of why each of the other three is wrong.
The twelve cases, in order
- Two diseases, one scope report
- What to give first, and why not steroids
- Scoring the colitis at the door
- The third day of the drip
- Pain, discharge and a tag
- The abdomen that stopped working
- The test before the tablet
- Screening before the biologic
- The centile that steroids cost
- The rash, the joint and the liver
- Colitis before the second birthday
- Vaccines before immunosuppression
What this deck teaches
- The rectum tells you which disease. Ulcerative colitis starts there and runs continuously; a spared rectum with ileal disease is Crohn.
- Food is the first drug in Crohn disease. Exclusive enteral nutrition heals the mucosa as well as steroids and grows the child instead of stunting him.
- Severity is a score, not an impression. PUCAI 65 or more is acute severe colitis – admit for intravenous steroids and involve the surgeons.
- Day 3 is a checkpoint. PUCAI above 45 on day 3 predicts steroid failure – prepare rescue therapy rather than waiting.
- Image and drain before immunosuppressing. Perianal Crohn gets MRI and examination under anaesthesia before anti-TNF, never after.
- Diarrhoea that stops while the child worsens. Falling stool frequency with distension and fever is toxic megacolon – stop antimotility drugs and call surgery.
Exam pearls from this deck
- A spared rectum with an inflamed ileum is Crohn disease, whatever the colon looks like.
- In a child with Crohn disease, food is the first drug – and the only one that grows him while it heals him.
